
WHAT IS THE MINOR HOME MODIFICATION PROGRAM?
The program provides safety equipment to assist consumers with personal care (grab bars in the bathroom, hand held showers, shower chairs, transferred benches, raised toilet seats are a few items that are provided), build ramps, extermination, handrails, door locks, doors, limited electrical and plumbing repairs, etc.
WHAT IS THE PURPOSE?
To promote safety in the home and allow the consumer to remain independent. Give the consumer emotional well being and prevent premature institutionalization.
HOW IS THE PROGRAM FUNDED?
The Minor Home Modification Program is funded through a federal grant provided by the Older American Act and the Medicaid Waiver program.
WHO IS ELIGIBLE?
Any person who is 60 years of age or older or disabled under the age of 60. No services will be denied on the ground of race, color, or national origin.
HOW MUCH DOES IT COST?
This service is free of charge to the consumer, but contributions are accepted.
WHAT AREAS ARE SERVED?
The Minor Home Modification Program serves consumers in eight counties, Carter, Greene, Hancock, Hawkins, Johnson, Sullivan, Unicoi and Washington.
HOW DO I QUALIFY FOR THIS PROGRAM?
A Registered Nurse (RN) will qualify consumers for the Medicaid Waiver program and an OPTIONS worker will qualify consumers for the Title III program.
The FTAAAD will determine which program the consumer qualifies under. They will forward the referral to the appropriate person or agency.
WHAT IS OPTIONS?
OPTIONS is a case-management program provided through the First Tennessee Area Agency on Aging and Disability (FTAAA&D). OPTIONS offices are located in the local senior centers throughout the eight counties area.
